Community Supports:
- Housing Transition Navigation Services, which assist individuals with obtaining housing (including assistance in searching and applying for housing).
- Housing Tenancy and Sustaining Services, which aim to help individuals maintain safe and stable tenancy once housing is secured.

Joining Community Supports Housing Transition Navigation Services under CalAIM offers a powerful set of benefits designed to help Medi-Cal members overcome housing barriers and achieve stable living situations.
Here is what you can expect:

- Personalized Housing Assessment to identify your preferences, needs, and barriers to successful tenancy
- A tailored housing support plan that fits your situation
- Assistance with locating appropriate housing options
- Support with applications, documentation, and eligibility requirements
- Benefit Advocacy
- Help navigating systems like SSI, SSDI, or other income supports
- Guidance on accessing housing vouchers and subsidies
- Support in Landlord Engagement
- Mediation and advocacy to improve your chances of securing housing
- Once housing is secured, you may be eligible for Tenancy & Sustaining Services: Ongoing support to help you stay housed, including budgeting, conflict resolution, and lease compliance.
- Services are aligned with your Enhanced Care Management (ECM) team to ensure holistic support across health and housing needs.

Who qualifiesfor Housing Navigation or Tenancy
under Community Supports?
To Qualify, patients must be enrolled in a Medi-Cal Health Plan with one of the Partner Plans listed below; and have at least one chronic physical, behavioral or developmental health need with the inability to self-manage; and experience or be at risk of homelessness.
