Programs

Blood Pressure Monitor Lending Program

Each participant in the Hypertension track will have a blood pressure monitor lent to them for the duration of the program. Utilizing a blood pressure monitor at home can help patients and their healthcare providers make decisions regarding their blood pressure. Patients will either receive their blood pressure monitor at our North Highlands clinic or our Franklin clinic, courtesy of SMUD and the American Heart Association.

Diabetes Prevention Program

The Diabetes Prevention Program uses CDC-approved curriculum to educate, support, and inspire participants with prediabetes or a past history of gestational diabetes. The program is one year in duration and includes lifestyle change management support, education on nutrition and physical activity, and social support provided by a group of peers. The program is facilitated by a CDC-certified Lifestyle Coach. Participants in this program have cut their risk of developing Type 2 diabetes by 58% (71% for people over 60 years old).

Franchesca Burgos, BSW serve patients as Lifestyle Coaches to help them prevent the onset of Type 2 diabetes.

Intensive Behavioral Therapy Program (IBT)

The CDC-certified Lifestyle Coach also runs a program called Intensive Behavioral Therapy for Obesity. This program is available to participants who have a BMI of 30 or above and who do not have prediabetes. The program is one year in duration and includes information on how to manage lifestyle modifications in physical activity, nutrition, and participants’ approaches to both.

Reach Out & Read

 

Reach out and Read is currently offered at our Halyard, 15th Street, Marysville, and North Highlands locations. We are working on expanding this valuable program to all our clinics that provide pediatric services.
